Missouri Health Insurance 2027: Rate Increases, Open Enrollment Dates, and How to Avoid Overpaying

If you buy your own health insurance in Missouri, your 2027 renewal is probably going to cost more. Every insurer selling Marketplace plans in the state has asked for a rate increase, and open enrollment for 2027 starts November 1.
This guide is for anyone in St. Louis, St. Charles County, or anywhere in Missouri who doesn't get health insurance through a big employer: self-employed folks, 1099 workers, small business owners, and anyone between jobs. Here's what's changing, what it means for you, and a simple plan to avoid overpaying.
How Much Is Missouri Health Insurance Going Up in 2027?
Missouri insurers proposed an average increase of about 12.5% for 2027 Marketplace plans, before any subsidies. Nationally, the typical requested increase is around 15%. If those go through, Marketplace premiums will have climbed more than a third in just two years.

Small businesses aren't spared either. Group health plans for companies with 2 to 50 employees are projected to rise between 9% and 19%.
One more thing to know: six insurers will offer Marketplace plans in Missouri for 2027, and one company that offered plans this year is moving off the exchange. If that's your carrier, your plan could change even if you do nothing.
Why Are Health Insurance Premiums Rising?
The extra federal subsidies that kept premiums low the last few years expired. When costs went up, a lot of healthier people dropped their coverage. Missouri Marketplace enrollment fell about 25% in one year, from roughly 347,000 people to 261,000. A smaller, sicker pool means higher prices for everyone left.
The "subsidy cliff" is also back. If your household earns more than 400% of the federal poverty level, you get zero help paying your premium. For 2027 coverage, that line is roughly $63,840 a year for a single person and $132,000 a year for a family of four.

This hits self-employed people hard. Earn one dollar over the line and you could pay full price. The good news: the income on your application is your estimated income for 2027, and things like retirement contributions can lower it. That's worth a conversation before you enroll.
Missouri Open Enrollment 2027: Key Dates
Missouri uses HealthCare.gov, so these are your dates:
November 1, 2026: Open enrollment starts, and 2027 plans and prices go live.
December 15, 2026: Enroll by this date for coverage starting January 1.
January 15, 2027: Last day to enroll. Coverage starts February 1.

After January 15, you can only enroll if you have a qualifying life event, like losing a job, getting married, having a baby, or moving.
What to Do Before Your Plan Auto-Renews
If you do nothing, HealthCare.gov will usually renew you into the same plan, or a similar one, at the new price. That's often the most expensive way to handle it. Instead:
Step 1: Open your renewal notice. Look at the new monthly premium and deductible, not just whether you're still covered.
Step 2: Update your 2027 income estimate. Your subsidy is based on it, and an old number can cost you money now or at tax time.
Step 3: Check your doctors and prescriptions. Networks change every year, even when the plan name stays the same.
Step 4: Compare at least two or three options. A different metal level or carrier can save hundreds a month.
Step 5: Look beyond the Marketplace if you're over the subsidy cliff. Off-exchange and private health plans may be a better fit for some healthy, higher-earning folks.
Step 6: Get it done by December 15 so your new coverage starts January 1 with no gap.

Frequently Asked Questions
When is open enrollment for 2027 in Missouri?
November 1, 2026 through January 15, 2027. Enroll by December 15 for coverage starting January 1.
How much will health insurance cost in Missouri in 2027?
Missouri insurers proposed an average increase of about 12.5% before subsidies. Your actual cost depends on your age, county, income, and plan.
Do I still qualify for a subsidy in 2027?
If your household income is under 400% of the federal poverty level (about $63,840 for a single person), you may. Above that, there's no subsidy.
Does it cost anything to use a health insurance broker?
No. Brokers are paid by the insurance company, so you pay the same premium whether you enroll on your own or with help.
What if my health plan is going away?
You'll get a notice, and you may be auto-enrolled in a different plan. Review it before December 15 so you're not stuck with something that doesn't fit.
What are my options if I'm self-employed in Missouri?
You can buy a Marketplace plan, an off-exchange plan, or in some cases a private plan outside the ACA. The right choice depends on your income, health, and doctors, which is exactly what a free rate check is for.
